National Lake Trout Derby 2017

I am trying to find information pertaining to this year's National Lake Trout Derby 2017. Their website doesn't have any information about this year's derby. I have seen information about the changes this year but I am mainly looking for the registration form for 2017. Any assistance would be greatly appreciated. Thank you

I scanned one of the entry forms in a sa jpeg. hopefully it will serve your purposes. They have stated that it is OK to photocopy entry form so you can make multiples if needed. I needed to block out my address etc. on it.
